Add QR code toggle to Personal Settings

In instructor personal settings, provide a toggle which allows the QR code to be displayed as a default, rather than starting a Poll Deck, selecting Settings in the control toolbar then toggling the QR code on.

I feel the QR code should be in my preferences but it isn't. Instead I can only turn the QR code on from my first echoPoll slides.
Once set, the QR code becomes default.

This would be a great feature in order to fix a QR code for a specific course, so all sessions are automatically added to the course associated with the QR code
